Practical support to partnerships across Scotland to deliver better health, housing and social care services
JIT is a strategic improvement partnership between the Scottish Government, NHS Scotland, CoSLA, the Third Sector, the Independent Sector, and the Housing Sector, governed by the recently formed Joint Improvement Partnership Board.
We aim to work together, on a voluntary basis, with partnerships to achieve real, sustainable improvements that suit local needs and to find practical solutions which translate into positive outcomes for patients, users and carers.
These are the key action areas to support service developments and improvements:
- Care at Home
- Delayed Discharge
- Equipment and Adaptations
- Governance and Management
- Integrated Transport with Care

Innovation to Transformation - JIT Annual Report 2012/13
2012/13 has seen progress on many fronts for the JIT. We have inspired, supported, challenged and coached 32 Reshaping Care partnerships to use their Change Fund to drive improvement in health and care with, and for, local people. We have spread knowledge and good practice, built positive relationships with and across all sectors and supported local partners to find shared resolve, use their resources more wisely and make greater impacts on individuals, their families and their communities.

National Telehealth and Telecare Delivery Plan
With technology playing an increasing role in our everyday lives, telehealthcare will be integrated into service development and delivery, transforming access to and availability of services in our homes and communities and more acute settings. Working in collaboration with the JIT, delivery will be supported by the Scottish Centre for Telehealth and Telecare.
