Community Care Outcomes for Joint Working

JIT's main focus is implementation, although we're closely linked to policy development teams within Scottish Government.

What does Joint Outcomes mean?

In simple terms, Joint Outcomes refers to the policy area that focuses on improved joint working between local authorities and NHS health boards in Scotland as they deliver community care services. The main reason for developing and implementing Joint Outcomes initiatives is to provide faster, better, safer, and more local access to services by helping agencies deliver services jointly.

As the name suggests, Joint Outcomes is outcomes focused. This is a key aspect of JIT's work, as our remit is to provide practical support that helps health and social care partnerships deliver better outcomes.

What is the Community Care Outcomes Framework?

The Community Care Outcomes Framework includes 4 national outcomes and 16 performance measures. National targets are also currently being developed as part of the Framework.

The four national outcomes are:

  • Improved health
  • Improved wellbeing
  • Improved social inclusion
  • Improved independence and responsibility

The 16 performance measures cover users' satisfaction with services, waiting times, quality of assessment, shifting the balance of care, carers' well-being, unscheduled care and identifying 'people at risk'.

The Joint Outcomes website has an area dedicated to National Outcomes, which is regularly updated with the latest information. It provides information about how the National Outcomes Framework is being developed and implemented, and how outcomes will be measured.
