Integrated management and resourcing across CHCP for the provision of social and specialist health care for people with learning disabilities.   Developing services in line with “The Same As You?” and subsequent “Keys to Life” strategies in terms of joined-up, person-centred and community-based supports and services for people with learning disabilities.  Strategic fit with partnership agency approaches, e.g., NHS Greater Glasgow and Clyde service modernisation programme and East Renfrewshire Partnership in Practice and Learning Disability Strategy.

Barrhead Health and Care Centre

Examples of Practice

Re-provision of dated health and social care premises with an integrated health and social care centre.  Prior to the development of the integrated health and care centre, CHCP social work teams were located in three additional buildings which were dated and assessed as not fit for purpose.   The previous Health Centre dated from early 1980s, did not house all practices within the locality and had restricted space which constrained the development of local services.

Performance Management in Argyll & Bute

We intended to gather management information into a single format that could inform and improve our performance across the partnership, bringing together health and social care information, to allow managers to work together to bring about change and re shape care in the area. We also developed and applied service user and carer questionnaires using the Talking Points approach.

Orkney Health and Care – Evaluation Outcomes

Examples of Practice

Orkney Health and Care (OHAC) has been under development as an integrated health and social care partnership since November 2010.  A restructure process has been undertaken to establish an integrated generic management structure, supported by a professional structure.  Staff within teams have been integrated and new joined up ways of working are in place.  An evaluation of the process and outcomes to date was undertaken in late 2013, with facilitation by the Scottish Social Services Council and NHS Education Scotland leadership development Action Learning Programme.
