Service Working to Integrate Therapy into Community Health (SWITCH)
The SWITCH Partnership is a four year programme responding to the Government's modernisation agenda by completing an evidence based service redesign within occupational therapy older adult services in NHS Lanarkshire and South Lanarkshire Council. The findings from the Baseline Evidence and Service Redesign Planning report will help shape the next step in the process. You can read a brief overview of the programme as well as a summary of the baseline findings from June 2008.
