Reshaping Care Improvement Network
What?
The Scottish Government’s policy on Reshaping Care for Older People aims to optimise independence and wellbeing for older people at home or in a homely setting. The implications of the current financial situation and demographic changes make this a challenging task, as improved services need to be provided for an increasing number of people using a diminishing level of resources.
The Joint Improvement Team (JIT) have established an Improvement Network to support partnerships to realise their Reshaping Care ambitions as they implement their Change Plans. The Network purpose is to:
- Support delivery of the £70m Change Fund programme and reshaping care in the context of the £4.5bn currently invested across health, housing and social care on behalf on older people;
- Facilitate full and active partnership engagement and integration in reshaping care for older people, particularly within the housing, independent and third sectors and within health in general;
- Support all 32 partnerships to test and spread local improvements and share learning;
- Increase the pace of change;
- Support the use of measurement and collect and share evidence of the impact of reshaping care and the change fund;
- Work closely with other work streams and improvement agencies to ensure a coherent approach to supporting the implementation of change fund and reshaping care programmes.
Who?
The Improvement Network is a collaboration between JIT, the Quality and Efficiency Support Team (QuEST) and the Scottish Community Care Benchmarking Network. Together, they also work with a range of stakeholder organisations and improvement partners and will engage existing communities of practitioners.
How?
In addition to named JIT leads as the principal point of contact for each partnership and linkage with wider JIT work, the network will provide support to partnerships through:
- Programme of regular WebEx virtual meetings
- Programme of national and local events
- Publishing regular e-bulletins
- Regional Action Learning Sets - supported by WebEx, local JIT Action Group members (initially), and local events as above
- Evidence, resources and improvement tools on website
- Case Study Highlights, signposting to support
- Resources Guidance
- Training on improvement methodologies appropriate to Reshaping Care, linking to improvement tools through Quality Hub, Improvement Service etc.
- Linking with existing Communities of Practice and supporting communities of practice
- Use of measurement for improvement
- Collaborating across programmes and work streams
- Integrating outcomes-based approaches across a range of activities
Case Study Highlights
The Case Study Highlight documents are designed to be quick and easy read documents showing what experiences have been recorded elsewhere and how the reader can use these experiences to shape their own thinking and services. They are not full case studies and will have contact information to encourage networking and sharing or knowledge. These are organised by pathways for ease of access.
Shared Outcomes
Success in Reshaping Care is being evidenced through an agreed set of Reshaping Core Improvement Measures. These Core Measures are aligned with, and will support the delivery of the Quality Strategy Ambitions, and will form part of the Quality Measurement Framework. They are equally aligned and integrated on-going work to review the Community Care Outcomes Framework. The measures fall into three groups:
- Nationally available outcome measures and indicators which use data already collected at local level and compiled nationally
- A set of local improvement measures to inform local change
- Measures of shift in Partnership resource and in Change Fund use over time
