Demonstrators of Home-Based Telemonitoring to Support Self-Care in Long Term Conditions

Three health and social care partnerships - Lanarkshire, Lothian and Argyll & Bute - have been funded through the programme to implement projects aimed at supporting people with long-term conditions to manage their symptoms and disease more effectively at home. The initial focus for all three projects is COPD (Chronic Obstructive Pulmonary Disease), but with plans to expand to support people with other conditions and within other home-based settings (sheltered housing, for example).

Argyll and Bute published the following evaluation of their findings in August 2010.
