This is an Example of Practice illustrating how initiatives and extra services run by Housing Associations directly or indirectly have a significant positive impact on the quality of life, health and well-being of those people being supported.

This example describes the range of supportive activities and resources provided by an Extra Care Team to new and established tenants, most of which are targeted at more vulnerable tenants, and looks at the impact of these. The connection between these activities and health and/ or well-being is not always immediately obvious, but nevertheless exist.

This example describes the practical assistance provided to tenants by Muirhouse through its Enhanced Management Service. This service aims to help people maintain their tenancy, enable people to improve the quality of their life and create the conditions in which people may be more able to address aspects of their health and well-being.

This an Examples of Practice illustrating how initiatives and ways of working in housing, housing support and homelessness services have significant positive impact on the health and wellbeing of those people being supported and contribute towards the nine national health and wellbeing outcomes for Integration.

Penumbra wished to transform its services in Aberdeen from existing shared, supported accommodation to a more flexible, personalised approach which offered greater opportunity for ordinary living with each person having their own flat.  They considered that this model would promote recovery.

The challenges included shifting from institutional based care to care at home or a homely setting, the need for quicker assessment and care plan coordination and formulation in a more homely setting taking into account the person’s conditions and home situation instead of needs and support being assessed in a ward setting.
