Timely adaptations including housing adaptations

Examples of Practice

Waiting times for Occupational Therapy (OT) assessment were increasing and consultation with OT staff across Angus identified that they were spending a significant amount of their time in the office dealing with general OT enquiries, taking and prioritising referrals and dealing with service user complaints about delays. This prevented staff from seeing service users and arranging essential equipment and adaptations. The process for dealing with some minor adaptations had also been identified as cumbersome and time consuming for staff and resulted in delays in the adaptation being put in place.

Examples of Practice

Falls are the most common call received by the Scottish Ambulance Service in Aberdeen, with a mid-morning peak.   The purpose of this project was to maximise the number of Older People who can be supported to remain at home, following a fall.  This is intended to significantly reduce the number of unnecessary hospital admissions for older people.

Alternative Housing Models and Intermediate Care

Examples of Practice

Issues the projects addressed were:

  • Supporting people to live more independently in the community for longer, thereby reducing care home admissions
  • Making better use of care services to move away from the rigid model of home carers visiting
  • Working with Registered Social Landlords to bring together the Housing Support function with personal care
  • Filling the gap of reduced social and community based activities as a result of the move from institutional models of care

Older People’s Access Line (OPAL)

Examples of Practice

The project allows early intervention with customers providing a range of services aimed at decreasing their reliance on medical or other statutory provision.  Many of the customers were socially isolated and did not know how or where to access services and community supports OPAL allows for the customer to gain information about local social activities, lunch clubs and advice and information on such issues as benefits, income generation, housing, support for carers and volunteering.
