Traditionally exercise drop off rates for exercise referral schemes across the UK are very high and reported at 75-80% across the industry post or during the 12 week brief intervention. The aim of Active Living Express is to achieve at least 50% follow on rate which will be achieved through the elements highlighted below.

Prior to the establishment of the Duty and Response Team, there was no community rehabilitation team and social care and health elements of support operated independently, albeit collaboratively with one another. This still meant duplication of information sharing and assessment processes for older people requiring the service. The Change Fund enabled the addition of Physiotherapy and Occupational therapy professionals to join the Adult Wellbeing duty and response team to form a multidisciplinary duty, response and rehabilitation team.
Increasing the supply of housing suitable for older people is a key priority in the South Lanarkshire Local Housing Strategy, 2012 – 2017, both in terms of meeting current demand and the significant projected increase in the 65+ population. At 31 March 2013, there were 2177 applicants on the council’s housing register for sheltered and amenity housing, with only 273 properties becoming available for let in 2012/13.
