The evidence base to inform the allocation of the Change Fund was acknowledged to be weak in many areas, and a particular gap was identified in relation to the factors that build and maintain the resilience of older people in the community. Making it Clear was commissioned to build evidence in this area and also to apply the findings in a practical way.
Investment to make improvements in the community equipment store to ensure systems are in place to provide outcome focused assessment and the provision of an effective equipment service – This is recognised as the responsibility of all care groups and services as a means of complementing other service provision, interventions and supporting overall service delivery e.g. facilitating hospital discharge. The Joint Equipment Store was working at full capacity to accommodate the increasing numbers of equipment requests and increasing costs as we strived to support more people in their own homes.
In the view of ‘Care closer to home ’(Darzi) 2008, AHPs play an integral role in deciding whether a patient is safe for discharge. They are well positioned to deal with the wide range of pathologies and complexities presented. Additionally, evidence relating to emergency admissions of older people suggests assessment should take place as early as possible depending on medical stability (RCPL – Management of the older medical patient, RCPL 2000).
The aims of this project are to support the independent sector to embrace the challenges and opportunities in transforming support for older people as required by the Reshaping Care for Older People (RCOP) agenda. The project will seek to identify how providers can shape their delivery of care and support to meet local RCOP joint strategic commissioning plans and deliver improved outcomes for older people. This project will develop practical strategies/tools based on the needs and experiences of providers and share them with the wider independent sector. Selected independent providers will be closely involved participants throughout the project.
