What was the issue you were addressing or working on?

Improve networking with staff working as part of the Integrated Community Support Team and increase knowledge of Carer support services including information website/s.


What you did?

South Lanarkshire Carers Network appointed a Carer Resource Development Officer with a capacity building role in relation to Carer support. One of the responsibilities is to link with the staff involved in the Integrated Community Support Team. The purpose of this is to raise awareness of Carer issues and local support services that are available. The aim is to improve the identification of Carers and facilitate effective signposting/ referral for support.

A timetable of team presentations and contact has provided the opportunity to share information about support services for Carers such as information pack, GP register, emergency cards, welfare rights, local carers centre (PRT) and other community projects. Information about the carers profiling and assessment process was also shared and discussed. Individual Carer champions have also been identified.


What were the outcomes - benefits or otherwise?

Effective working relationships are established and Carers looking after a relative supported by the ICST are provided with information. The team has a link person for any Carer enquiries/issues. To date referrals have included Carers requiring assessment of their support needs, money & benefits advice, training in relation to their caring role, short breaks from caring, support in relation to hospital admission and discharge and on-going individual support.
