Immediate Discharge Service – Reablement, Perth & Kinross (Update)

Examples of Practice

The number of people aged over 65 living in Perth and Kinross is expected to increase by 74% by 2031.  The Reablement Service launched in Perth City on Monday 4 October, 2010.  The service is aimed to help people regain skills that will allow them to live as independently as possible in their own homes.  The Reablement service is also key in ensuring the Council can meet the challenges that lie ahead for service provision.

The Zone Initiative Home Buddy Scheme – East Ayrshire- Update

Examples of Practice

The Zone Initiative Home Buddy Scheme was launched in 2010 and provides a domestic and household service for older, disabled and vulnerable people in the Doon Valley area.  The Home Buddy Scheme is an intergenerational project providing a personal and professional service which is individually tailored to assist people to live in their homes and communities for longer without feeling lonely, isolated or afraid.

The service is funded by the Henry Smith Trust and the Reshaping Care for Older People- Change Fund and has 156 older clients.

Community Engagement, Resilience and Health Service Development in Annandale and Eskdale (CERAE)- Update

CERAE represents an extension of the role of the Safe and Healthy Action Partnership (SHAP) in Annadale and Eskdale.  SHAP is a partnership of various organisations from all sectors (public, private and third) which seeks to improve the health and well-being of people in the locality.

Examples of Practice

This an Example of Practice illustrating how initiatives and ways of working in housing, housing support and homelessness services have significant positive impact on the health and wellbeing of those people being supported and contribute towards the nine national health and wellbeing outcomes for Integration.

This example focusses on the outcomes being achieved through the pathway created by its Direct Access and Resettlement Service in Glasgow and centres on James who had been evicted from his tenancy and stayed with a friend before moving into Aspire’s Direct Access Service.  He had a problem with budgeting and issues with addiction, consuming on average 10 pints of lager per day. James did not initially perceive his alcohol consumption as being an issue. However, over time he decided that it was something he would like to address.

Adult Befriending Service, Orkney (Update)

Examples of Practice

The health and social care partners in Orkney recognise there is significant evidence to show that isolation can both cause and exacerbate poor health outcomes.  Reducing isolation is understood to prevent or at least delay such outcomes from arising and therefore prevent or at least delay the increased use of other more expensive services.  The Befriending Service therefore forms an integral part of Orkney’s overall approach to the improved health and well-being of older people.
