Neighbourhood Links and Red Cross Buddies (Scottish Borders)

Examples of Practice

The British Red Cross Neighbourhood Links and Buddies service provides support to people with low level social needs by assisting them to re-engage with their community and provides them with information to enable them to better manage their day to day activities.

The project supports both people in existing community settings as well as those requiring safe early discharge from hospital through helping clients to re-settle in their homes following an episode in hospital or intermediate care.

British Red Cross Patient Transport Service (PTS) and Kytton’s Guest House

Examples of Practice

The PTS (Patient Transport Service) accepts referrals from a range of sources, including hospitals, GPs social workers, intermediate care teams, patients, families and carers. It transports older people who would, for various practical reasons, find it extremely difficult to get from one place to another but whose need is not of the type that requires an ambulance. Typical journeys will be from home to hospital and transfers to the ferry and air ports when hospital appointments are on the mainland.

Aberdeen City Befriending Partnership

Examples of Practice

The potential ill effects of isolation, loneliness and reduced independence are well documented and can be acute, with compromised physical health due to inactivity and poor mental health being two very particular risks. The Aberdeen City Reshaping Care for Older People partners are very aware of this. ‘Reducing isolation and loneliness’ and ‘optimis(ing) independence’ remain key priorities in their Change Plans, hence their unequivocal backing for the ACBP.

Through its support, the ACBP will seek to directly address these priorities and help prevent the poor health and can otherwise result. The ACBP wants to help ensure that older people’s lives are about ‘living well not just living’.

Aberdeenshire Signposting Project

Examples of Practice

The Aberdeenshire Signposting Project works by linking people to local organisations, services, clubs and societies in their community that address issues in a person’s life.  The Project had become aware of growing numbers of older people at risk of social isolation resulting from life events such as bereavement, move of house because of physical needs, or impact of developing chronic illness.  These older people were presenting at GP surgeries with a range of conditions.
