Delivery Framework for Adult Rehabilitation

The Framework focuses on the rehabilitation needs of older people, people with long term conditions and vocational rehabilitation and is explicitly linked to existing work streams of anticipatory care, unscheduled care, planned care, self managed care and the management of all long term conditions. The Framework will impact in key areas such as the prevention of unnecessary admissions, better identification and management of the ‘at risk’ population, safe early discharge, the prevention of dependency on care services and the health benefits of returning to work. The Framework is also integral to the implementation of the Strategy for Community Hospitals and Shifting the Balance of Care.

NHS Lanarkshire Intermediate Care Development: Event Report

In July 2006 NHS Lanarkshire attracted funding from NHS Education for Scotland (NES) for a four-year project, which will focus on intermediate care for older people. This project aims to develop and implement an interprofessional and interagency educational programme and competency framework which will support practitioners working with frail older people in multidisciplinary health and social care teams across acute and community settings.

On the 6th of October 2006 a Development Event was held to formally launch the project and begin to profile Lanarkshire’s intermediate care services. This report outlines the main findings of this event and details the next steps to be undertaken.

Intermediate Care for Older People with Mental Health Difficulties: Early Findings Paper

This paper presents the standards and the early findings from a Department of Health funded national project on Intermediate Care for Older People with Mental Health difficulties. The project benchmarked the intermediate care service provision in seven sites across England during March and April 2006 against best practice standards developed by the Health and Social Care Advisory Service (HASCAS).
