The aims of this project are to support the independent sector to embrace the challenges and opportunities in transforming support for older people as required by the Reshaping Care for Older People (RCOP) agenda.  The project will seek to identify how providers can shape their delivery of care and support to meet local RCOP joint strategic commissioning plans and deliver improved outcomes for older people.  This project will develop practical strategies/tools based on the needs and experiences of providers and share them with the wider independent sector.  Selected independent providers will be closely involved participants throughout the project.

Telecare in Care Homes, Lothian

To reduce the level of emergency admission to hospital from Care Homes by utilising appropriate telecare equipment to reduce the number of falls in residents.

Older People’s Advice Service (OPAS), operated by Link Housing Association

Examples of Practice

To combat poverty and financial exclusion by improving take-up of older people’s benefits with particular emphasis on clients who have health problems, are housebound, or who live in remote areas.

The service aims to ensure that hard to reach older people are given an opportunity to claim the benefits and access the support that they require.  Typically older people do not access benefits and support due to multiple barriers.  The OPAS delivery model aims to overcome these barriers.

The project focused on Falkirk, Clackmannanshire, West Lothian, North Lanarkshire and Glasgow.
