Instructions

  1. Before you can use the Partnership Outcomes Performance Improvement Tool, you must first register using the registration form.
  2. The questions are arranged in 4 sections. Once registered, you can exit and save your answers at any time. When you return to the website and login, you will resume at the place you left off. This also allows you to return to unanswered questions later.
  3. To move between sections, use the tabs shown above the questions.
  4. To generate the final report, clicking on the ‘Generate Report’ tab to view findings and recommendations based on your answers. This report can be saved as a web page or viewed at any time by returning to these pages and logging in.

Note:
If you are not registered you can view all the question in the 4 sections, but you will be unable to answer any questions and generate a report.
