1The partnership has agreed a set of outcomes for individuals that we are aiming to improve.
2These outcomes are clearly described so that everyone knows and understands what they are.
3When we were identifying these outcomes, we used information from individual outcome-focussed assessments, care plans and reviews, and surveys, to identify how we can jointly improve our impact on the lives and well being of people who use services, patients and carers (their individual outcomes).
4Every individual and organisation in the partnership is committed to improving the agreed outcomes and this is reflected in their own objectives.
5We can measure these outcomes so that we will know when they have been achieved.
6The outcomes our partnership is working to improve for people are reflected in the Single Outcome Agreement for the Community Planning Partnership.
7We understand how our agreed outcomes are consistent with the Scottish Government’s Performance Framework and the Community Care Outcomes Framework.
8Staff and teams have the authority to change the way services are delivered or are commissioned if they get in the way of improving outcomes for people.
9We have an explicit, agreed set of behaviours expected from partnership participants.
10We have developed an atmosphere of trust, loyalty, commitment and open challenge in the partnership.
11All members of our partnership are enthusiastic about what can be achieved by working together with other agencies in the partnership.
12The partnership supports outcomes improvement with a planned, funded programme of organisational development for all levels of the partnership.

Linking Partnership Actions to Outcomes

Measurement and Monitoring Systems

1The outcomes being achieved by people who use services or support, patients and carers are recorded in a systematic way.
2Measures of outcomes used are agreed across agencies so that everyone is speaking the same 'language' when these measures are brought together.
3There are efforts to ensure that staff time in inputting information is minimised wherever possible.
4Performance information that we review regularly in our partnership relates directly to the impacts of our services and support on the lives and well being of people who use services / patients and their carers.
5When we achieve improvements in outcomes for people, these successes are celebrated in our partnership.
6A variety of formats for presenting information is used, including qualitative measures such as individual people’s stories as well as collated, quantitative evidence of the effectiveness of services and support.
7The information that we receive helps us draw conclusions about what is and what is not working so that we can make changes if necessary.

External Reporting and Scrutiny

1There is clear linkage between performance reporting to our partnership and the reports which are received by each of the individual member organisations.

4Outcomes focussed performance information is regularly used to give staff feedback on successes and areas where improvement is needed.
5The partnership learns from others by sharing information and experience in local and regional networks.

7There is an annual review of the effectiveness of the partnership's way of working.
8Our partnership has a clear understanding of the inspection, quality and accreditation frameworks within which each individual partner operates and incorporates information reporting requirements for this into partnership reporting.
9Each agency in the partnership uses the partnership’s outcome-based performance information to inform its self-assessment, which is in turn provided to the relevant scrutiny body/bodies.
10Our partnership embeds the improvements identified in self-evaluation action plans and requirements made by external scrutiny bodies into partnership action plans.
11There are arrangements in place to engage people who use services and/or support, and their carers, in monitoring and assessing our performance in improving outcomes overall as well as at an individual level.
12We have a process in place to identify, measure and review risks to the improvements in outcomes that our partnership has agreed.
