Care at Home Support Programme

Developing Care at Home Services is a crucial part of the Reshaping Care for Older People programme and the JIT is creating a support programme to assist local partnerships with service reviews and redesigns.

The thrust of the programme will be to help shift the emphasis on care at home services to re-ablement and rehabilitation models that seek to optimise the capabilities of older people.

Home Care - Re-ablement Programme

An initial focus for our programme will be to support local partnerships to review and redesign home care services to a re-ablement model.

There is a lot of interest in re-ablement approaches across Scotland and a recent survey carried out by the JIT/ADSW Home Care Group indicates the progress being made. The survey report and position of each council can be accessed below.

Edinburgh Council has implemented a comprehensive re-ablement home care redesign and this has been independently evaluated by RRM Associates. The evaluation report indicates substantial quality and cost benefits. You can access the evaluation report and other material within the Care at Home Publications Section

Practical Support

In order to provide practical support to partnerships who are considering or who have started to redesign home care services JIT is arranging a series of regionally based 2 day workshops. The first workshops were held in Aberdeen (during April/ May 2010), these will be continuing in other regions over the next 6 months.

Following the workshops, the JIT will be able to provide up to 2 days “on site” practical support to local “redesign” teams if required and further inputs may be available by negotiation. We will also be setting up a telephone helpline and a web based Q+A forum. If there is interest, we will also support a Care at Home learning network.

This programme will be led by Alex Davidson (JIT Associate) and Gerry Graham (JIT Action Group) supported by Brian Spence (JIT core team).
